Targeted Delivery of Drugs to Brain Tumors

Trudy Forte of Berkeley Lab’s Life Sciences Division will discuss her work developing nano-sized low-density lipoprotein (LDL) particles that can be used as a safe and effective means of delivering anticancer drugs to brain tumors, particularly glioblastoma multiforme. This is the most common malignant brain tumor in adults and one of the deadliest forms of cancer. Her research team found that the synthetic LDL particles can target and kill such tumors cells in vitro. The nanoparticles are composed of a lipid core surrounded by a peptide. The peptide contains an amino acid sequence that recognizes the LDL receptor, and the lipid core has the ability to accumulate anti-cancer drugs. Her talk was presented June 27, 2007.

Outsmarting Cancer – Targeted Drugs Fight Lung Cancer in North Shore Patient

When Wilmington resident Joan O’Beirne, 68, started experiencing shortness of breath during her daily activities, she had no idea what could be causing it or that she was about to become part of a pioneering research study. A widow with three grown children, O’Beirne has always been active and healthy, never smoking, playing tennis, and more recently watching her grandchildren every afternoon. When she noticed her shortness of breath, she consulted her primary care doctor who sent her to a lung specialist. Her early tests were inconclusive; she showed no sign of chronic obstructive pulmonary disease. Seeking an answer, O’Beirne’s doctors ordered additional testing for cardiac disease and blood clots as well as a CT scan of her lungs. Finally, the CT showed scar tissue in the upper left lobe of her lung that a radiologist suspected might be a sign of cancer. A PET scan confirmed active cancer cells and she was sent to a lung surgeon for a biopsy to determine the extent of her cancer. After a surgeon advised that her tumor would require a surgical biopsy—a procedure that increased the possibility of spreading the cancer—O’Beirne sought a second opinion. A review of recent clinical trials on various targeted therapies for malignant mesothelioma indicates that, while some are helpful, none are likely to become standard treatments yet.

In an article in the international journal Cancer Chemotherapy and Pharmacology, a pair of Danish researchers reported their analysis of 32 clinical trials on 17 different targeted agents for mesothelioma. A targeted agent is a medication that takes aim at a specific cellular function within mesothelioma tumor cells. Some inhibit proteins needed for growth or replication. Others may attack the formation of blood vessels or other structures that feed the cancer cells.

The extensive look at clinical trials of these targeted agents found that, overall, those that were used as a first-line treatment for mesothelioma induced responses ranging from 0 to 14%. Those that were used as a second-line treatment, after another therapy had already been tried, induced a 0 to 16% response. One of the more effective targeted agents appears to be sunitinib (brand name Sutent), a tyrosine kinase inhibitor that triggered some tumor shrinkage in 10% of mesothelioma patients, but resulted in ‘stable disease’ for 66% of them when used as a second-line treatment.

Another potentially effective targeted agent for some mesothelioma patients is bevacizumab (brand name Avastin). Bevacizumab is a monoclonal antibody and the first FDA-approved therapy designed to inhibit angiogenesis, the process by which new blood vessels form to support cancer cells. Primarily used for the treatment of metastatic colorectal cancer, recurrent metastatic non-squamous non-small cell lung cancer, and metastatic renal cell carcinoma, it is being tested as an addition to chemotherapy for mesothelioma patients.

As reported by the Danish researchers, some of the results of Avastin trials have been positive. When added to pemetrexed and cisplatin chemotherapy drugs, bevacizumab ‘significantly improved disease control in 73.5% of mesothelioma patients, compared to 43.2% who had chemotherapy alone. Bevacizumab did not appear to offer the same advantages when used along with another chemotherapy combination – gemcitabine and cisplatin. Unfortunately, other studies suggest that the drug may increase the risk of kidney damage.

The results of these mesothelioma trials prompted the authors of the meta-analysis to cautiously conclude, “Disease stabilization is reported in some patients with several targeted treatments and might be beneficial in subgroups of patients or in combination with classic chemotherapy.” They add, however, that none of the current targeted treatments available are effective enough to be recommended as standard treatment for malignant mesothelioma.

INTRODUCTION

Mesenchymal-epithelial transition (Met) tyrosine kinase is the receptor for hepatocyte growth factor (HGF), which is normally active in wound healing, liver regeneration, and embryonic development. However, the Met pathway is one of the most frequently dysregulated pathways in human cancer. Met dysregulation leads to constitutive activation of its tyrosine kinase activity thus increasing the downstream signals that lead to cell survival, proliferation, and migration. Patients with upregulated Met tend to have aggressive, chemo-resistant cancers and poor prognoses. For these reasons, the pharmaceutical industry has expended much time and effort to identify potential Met tyrosine kinase inhibitors as potential therapeutics for these cancers.

POTENTIAL THERAPEUTICS

CrizotinibThe drug Crizotinib has been discussed previously in its role as an EML4-ALK inhibitor for treating non-small cell lung carcinoma. However, Crizotinib is also a Met-inhibitor and shows promise in several other forms of histological cancer associated with Met-dysregulation.

ForetinibForetinib is an inhibitor of both Met and vascular endothelial growth factor receptor (VEGFR). Foretinib completed Phase I clinical trials in 2010 and showed potential in treating aggressive solid tumors. Phase II clinical trials are currently underway testing its efficacy in both lung and breast cancer.

CabozantinibLike foretinib, Cabozantinib inhibits both Met and VEGFR. It was granted orphan drug status by the FDA in 2011. The orphan drug status is available to encourage pharmaceutical companies to develop drugs for diseases, known as orphan diseases, which have a small market because they are relatively rare. Orphan drugs proceed through their development along the same regulatory pathways as other drugs with the exception that some statistical burdens may be lessened in recognition of the limited patient pool. Furthermore, other financial incentives are also linked to orphan drugs such as tax incentives and extended patent protection. These exceptions are in place to encourage the pharmaceutical industry to develop drugs for these rare diseases, which, by definition, would have a small market share and be relatively unprofitable.

Cabozanitib is currently in clinical trials for treatment of prostate, ovarian, brain, melanoma, breast, non-small cell lung, hepatocellular, kidney, and medullary thyroid cancer.

SUMMARY

The Met tyrosine kinase is one of the most dysregulated pathways in many cancers. The use of c-Met inhibitors with other therapeutic agents could be crucial for overcoming potential resistance as well as for improving overall clinical benefit in multiple types of cancer. Therefore, for more than two decades, investigators have been exploring Met inhibitors as potential cancer treatments. Three drugs currently in development are crizotinib, foretinib, and cabozantinib, all of which are currently in early phase clinical trials for treatment of several different cancers. Met inhibitors may prove to be a targeted type therapy with reduced toxicity, that are effective at halting metastasis and improving prognosis in many types of previously resistant tumors.

AACR 2011: New Phase I targeted cancer drugs.

Dr Lori Friedman – Genentech, California, USA Dr Lori Friedman, director of cancer signalling and translational oncology at Genentech, speaks about a session at the 2011 AACR congress where a number of previously undisclosed drugs in Stage I clinical trials were presented. The first drug discussed was a molecule which targets FGFR1, FGFR2 and FGFR3 in order to combat gastric and haematological malignancies. The second is a highly specific small molecule to target HER2. This drug is particularly exciting as it is able to cross from the blood into the brain. Due to this property it may be useful in treatment of HER2 positive breast cancer that has metastasised into the brain. Finally Dr Friedman talks about a drug which targets the ALK gene responsible for a subset of non-small lung cancer. Pfizer have previously developed a first generation drug to target both c-MET and ALK but this second generation drug is more specific and targets only ALK.

The researchers at the Asbestos Disease Research Institute (ADRI) discovered a marker in the blood of mesothelioma patients that shows the aggressiveness of the disease. The rates of tumor cell growth can differ significantly patient-to-patient so the best method of treatment for one patient may not yield optimal results in another patient. This new biomarker test will allow physicians to personalize treatment plans, catering to each patient’s condition.

The biomarker test is a biological molecule that is found in the patient’s blood, tissues or other body fluids that indicates the presence or absence of a particular disease/condition, as well as its level of aggressiveness as far as cancer cell growth. Biomarker tests are useful in diagnosing and determining the treatment of mesothelioma. Researchers have been spending an immense amount of time and money in researching ways to improve the diagnosis and treatment of mesothelioma. Researchers also hope their efforts will improve mesothelioma patients’ survival rates through their ability to personalize treatment plans.

Providing mesothelioma patients with personalized treatment can optimize the potential for a successful remission. Mesothelioma treatments often differ dramatically in type and dosage. Biomarker tests determine the patient’s level of disease aggressiveness and can open doors to treatment methods that were not considered before or would have been considered otherwise.

ADRI’s research on the biomarker test was led by Professor Nico van Zandwijk who believed that this new approach may spare patients who have advanced mesothelioma and a poor prognosis from having to undergo useless chemotherapy or radical surgery. This saves patients not only a lot of time and money, but also a lot of energy.Mesothelioma researchers and attorneys are providing patients not only with medical options, but also legal guidance to ensure their rights are protected. Majority of the patients diagnosed with mesothelioma were exposed to asbestos either in the workplace or secondhand through the clothing of a loved one who did work around asbestos.

Conventional Mesothelioma cancer treatment consists mainly of three main options- surgery, chemotherapy, or radiation.

Newer medical therapies that are been discovered include the use of more targeted therapies that address the biological factors triggering mesothelioma growth might be a much more precise and effective way to combat this notoriously difficult cancer.

Mesothelioma is so difficult to treat properly because most often by the time it is diagnosed it is already in an advanced stage, and because it can take many biological forms. Current mesothelioma treatments were originally designed for other types of cancers, without considering factors that may be specific to mesothelioma. That may be why many patients only partially respond to treatment, and the average survival is just 12 months after diagnosis.

More specific “targeted therapies” that address the mechanisms that drive disease progression may actually provide a better means of prolonging the average life span of mesothelioma patients. “The term ‘targeted therapy’ refers to a new generation of anti-cancer drugs designed to interfere with a specific molecular ‘target,’ most often a protein or a receptor that is believed to have a critical role in tumor growth,” says lead study author Katalin Dobra, MD, PhD, a researcher in the Department of Laboratory Medicine at the Karolinska Institutet in Stockholm, Sweden.

“Targeted cancer therapy takes advantage of our recent knowledge of the key mechanism that the malignant [cancerous] cell needs for its survival, and uses the special properties of mesothelioma cells to design novel therapeutic strategies,” Dr. Dobra says.

The process starts with molecular screening to identify the characteristics of a tumor and how it behaves, what causes it to grow and spread. Then researchers test various therapies on cells in the laboratory to see if they can interfere with that process. Not until then can a new cure be tested on patients in clinical trials.

So far, Dr. Dobra’s team has identified three possible molecular targets, each with its own potential treatment:

* Proteasome-subunits are groups of enzymes that regulate cell progression and death. Potential treatment: Drugs called proteasome inhibitors have been shown to halt the growth of mesothelioma cells.

* The thioredoxin system contains components that are produced differently in cancerous and noncancerous mesothelial cells, and it is thought to contribute to the survival of mesothelioma cells. Potential treatment: In collaboration with other groups, Dr. Dobra has shown that the mineral selenite may trigger mesothelioma cell death while sparing healthy cells.

* Proteoglycans (PGs) are a class of proteins that differ based on the type of mesothelioma cell. They work by affecting the growth factors that help mesothelioma cells grow and divide. Potential treatment: Oligosaccharide sequences, which are made up of chains of sugar molecules, might interfere with the work of PGs.

One of these targeted approaches, proteasome inhibitors, is now being tested in clinical trials. The other two methods are also promising, but more research is needed before they can be tested on patients. Additional tumor targets are also being investigated.

The next step in the research is to combine various treatments-both traditional and experimental-to see which ones are most effective at targeting mesothelioma tumors, Dr. Dobra says. Attacking the tumors from as many different approaches as possible may offer the greatest benefit to mesothelioma patients.
