Chronic Pulmonary Obstructive Disease (COPD)…need the medical view?

what are the: aetiology, pathogeneis, morphology of this disease?
i want to ask the same question as above but this time about emphysemia and chronic bronchitis.
what are the medico-clinical importance of pulmonary function tests?
also what is the difference between restrictive and obstructive lung disease in respect of pathophysiology and histological findings?

Some of the most common causes of COPD are; smoking, industrial polution (from working in a coal factory etc), air pollution, only sometimes can it be inherited (this is very rare), old age, being male, allergy, repeated lung infections (bronchitus etc)
It takes from 3 months to a year to develop COPD and the morphology is a bit too confusing to explain!!
COPD is the general term for chronic bronchitus, emphysema and other illnesses so the above goes for both bronchitus and emphysema.
The difference between restrictive and obstructive lung disease is that restrictive lung disease is characterised by a loss of airway compliance (the ability of the lungs to stretch in a change in volume relative to an applied change in pressure. It is an important measurement in respiratory physiology) and COPD is characerised by an increase in airway resistance (factors limiting the amount of inhaled air to reach the alveoli)

Hope this helps a little bit. And I hope you know something about medicine or all the jargon I have used will be utterly pointless!!

A number of our Clients at Family inHome Caregiving of Monterey live with chronic obstructive pulmonary disease (COPD), a debilitating illness which impacts breathing and for which there is no cure. However, there are a number of things that one can do to make the disease more manageable, and less painful. The Monterey Herald had a great section in its Sunday edition of the fold out magazine americanprofile.com which can help the millions of people living with the progressive lung diseases such as chronic bronchitis and emphysema that block airflow and make it increasingly harder to breathe. More than 12 million people are diagnosed with COPD, but the American Lung Association says there may be twice that number of people because many people don’t know they have COPD. The seven expert approved steps they recommend to manage breathlessness and other symptoms to reduce the risk of sudden flare-ups (known as exacerbations) are:

GET AN ACCURATE DIAGNOSIS”People with COPD are way under-diagnosed,” says Dr. Stephen Rennard, a pulmonary specialist at the University of Nebraska Medical Center in Omaha. He recommends asking your Doctor to do a spirometry test which measures the amount of air you can inhale and exhale.

KICK THE SMOKING HABIT FOR GOODSmoking may not be the cause of every case of COPD, but it will make it get worst faster, says Dr. Neil Schacter, medical director of the respiratory care department of Mount Sinai Medical Center in New York.

MAINTAIN A HEALTHY BODY WEIGHTIf you are overweight, lose the extra weight because it will only make you even more short of breath. People with advanced COPD, however, are often underweight which is another problem that needs to be monitored.

GET A FLU SHOT”If you have COPD, the best prevention (for exacerbations) is to get an annual flu shot and a pneumonia shot every five to 10 years for people under 65,” says Schacter. The best time to get one is in late September or early October.

BE ACTIVEExercise improves your cardiovascular fitness and this allows you to do more if you have COPD. Build up to 30 to 60 minutes of brisk walking each day. Pulmonary rehabilitation, which includes exercise, breathing retraining, nutrition counseling and other supportive services, may also help.

CLEAN HOUSEDust, germs, fumes, animal dander and other irritants can aggravate COPD. Schacter recommends getting rid of carpets and drapes to make your home as antigen-free as possible.

RECOGNIZE THE WARNING SIGNS OF EXACERBATIONMore coughing, more phlegm, shortness of breath, feeling fatigued and not being able to do as much as before are signs of worsening COPD and you should see your Doctor if you notice any of these.

All of this is great advice. Many of those suffering from shortness of breath or other symptoms simply ignore it, getting a diagnosis can enable one to better deal with the problems of COPD.

The release of a large government study on Thursday resulted in some surprising results. Raising levels of the so-called “good cholesterol” (high-density lipoproteins or H.D.L.) may not be good for patients, contrary to what most Doctors have believed for many years. Studies which have tracked patients over time for decades have shown that those with higher levels of H.D.L. live longer and have fewer heart problems than those with low levels of H.D.L. Doctors, therefore, had thought that raising H.D. L. levels would benefit their patients, but that was not congruent with the results of this most recent study. “We were stunned, to say the least,” Dr. William Boden, a professor of medicine at the University at Buffalo told The New York Times. What shocked Boden and others which conducted the clinical trials is that patients taking niacin along with Zocor had higher levels of H.D.L. and lower levels of triglycerides, however, despite these improvements, the patients fared no better and may have done slightly worse than those taking Zocor alone. On Thursday, government scientists announced that Niaspan, an extended release form of niacin, did not provide any protection against heart attacks when taken with Zocor in patients with heart disease, and they slightly increased their risk of stroke. The news was bad news for Abbott Laboratories (it had Niaspan sales of 7 million last year and spent million on the study (the government spent another million)) but good news for many who had taken Niaspan or Niacin, many of whom suffered ill effects like flushing and headaches. A number of other companies including Merck & Company had been pouring millions into new drugs to cash in on what was being dubbed the “HDL hypothesis” which could be false. The companies had been trying to find new ways to exploit the market for cholesterol drugs after a number of statins (including Zocor in 2006 and Lipitor later this year) had expiring patents, which opened the market to generic drugs. There are a number of other related studies which are ongoing, including a Merck trial where 25,000 have been enrolled to take tredaptive, a niacin formulated with an agent intended to reduce the “hot flash-like” side effect many experience when taking Niacin. The recent study was dubbed “Aim-High” and followed 3,414 people for 32 months. It ended 18 months early because researchers decided there was almost no chance taking Niaspan would be beneficial. Some said the study was too small but Susan Shurin, acting director of the National Heart, Lung and Blood Institute, which sponsored the study, told the Wall Street Journal May 27 the study had sufficient statistical power to show whether Niaspan had a benefit in the high-risk patient population that was enrolled. It was halted after an analysis showed there was less than a 1 in 10,000 chance that a positive finding would result if the study went through 2012 as planned. The results show how important it is to continue testing drugs which come to market. There have been a number of cases in recent years where drugs have had to be pulled off the market after lethal side effects were found.

Google recently made a huge play, offering .5 bil. to buy Motorola Mobility, a 63% premium over its pre-announcement trading price. They’re said to be most interested in the company’s vast unexploited patent portfolio across a vast swath of technologies rather than the handset business. Similarly, drug companies have made billions upon billions of dollars off of proprietary patented products which debut as name-branded products and then eventually go generic. Although it’s understandable to protect proprietary formulas which will ensure a steady income for years and sometimes decades to come, some drug companies have come under fire for not sharing exciting breakthroughs with the public which could result in a cure for cancer or other diseases. The New York Times reported on August 25 that Myriad Genetics, which has a monopoly on the lucrative genetic tests for breast cancer risk, recently received a favorable ruling from the Federal Appeals Court which upheld its patent on two human genes and the validity on gene patents in general. Through its gene testing, which generated 3 million (88% of its 2 million in revenue) in its last fiscal year, the company has developed a huge database on the thousands of possible mutations in two genes which raise the risk of getting breast cancer. It used to cooperate with academic scientists trying to analyze the mutations as well as share this information with a public database maintained by the National Institutes of Health (NIH). However, a couple of years ago it stopped doing so to build its own proprietary database. Daniel Vorhaus, an attorney and the editor of the Genomics Law Report, said there were ethical questions regarding Myriad withholding the genetic information it has gathered as it is important for public health. Now that an appeals court has ruled that it’s possible to patent genes, a race will be on for drug companies to try to isolate and patent promising genome sequencing and lock out competitors. The Lance Oncology medical journal reported that cancer treatment costs are rising at such a rapid rate that they threaten to become unsustainable, even for wealthy countries. About 12 million people per year get cancer and costs associated with new cases was at least 6 billion in 2009. By 2030, about 22 million people will be diagnosed with the disease annually, according to a report released at a cancer conference held in Stockholm on Monday. As we come up with new treatment plans, they are becoming more and more complex–and costly. With state and federal government officials looking into every nook and cranny to find places to cut costs, extremely expensive treatments may get more difficult to have covered through insurance carriers, Medicare and Medi-Cal. Often times, if a treatment is new, insurance companies will deem them experimental and refuse to cover them, heart breaking for those for which it could be their last hope. “With an aging global population and an endless conveyor belt of expensive new drugs and technologies and increasing financial pressures, the cost of cancer care in high-income countries is becoming unsustainable,” said the article in the Lancet Oncology medical journal. A panel of economists, patient advocates, the journal’s editor and physicians met at the conference in order to try to find the best solution to treat cancer patients while not breaking the bank. “We are at a crossroads for affordable cancer care, where our choices, or refusal to make choices, will affect the lives of millions of people,” said Richard Sullivan, a professor at King’s College Integrated Cancer Centre in London. We are lucky to live in a developed country with good medical care, but I fear that as we struggle with an almost unprecedented financial crisis, the government will have to make hard choices about what they will pay and what seniors will pay. It’s likely that the out of pocket expense for seniors medical costs will rise significantly in coming years. It will be a shame if human lives are sacrificed because we can’t afford to support their medical costs. Already reports have surfaced that many people are skipping Doctor appointments because of the high cost of gas and the cost of medical care. I wrote recently on my blog about an exciting new breakthrough where researchers can use what they are calling “personalized medicine” to find patients which have a specific genetic mutation and treat only those with drugs which attack the cancerous cells. The first drug approved was a skin-cancer treatment which the Food & Drug Administration (FDA) approved last week. They were so excited about the potential that it was hurried through the approval process–it had only been submitted in May for approval. It was announced on August 26 that another drug was whisked through the system, this one to treat lung cancer. The twice per day pill treatment produced by Pfizer (Xalkori) was approved to treat a small subset of non-small cell lung cancer patients, less than 7 percent, who have an abnormal gene that stimulates cancer cells and causes tumor growth. It works by blocking proteins produced by the gene. “It’s another example of how we’re using molecular medicine to subtype lung cancer into more specific and treatable diseases,” said Dr. Roy Herbst, a lung cancer expert who is chief of medical oncology at Yale University. The FDA said it also approved a genetic test made by Abbott Labaoratories to screen for the mutation, known as an abnormal anaplastic lymphoma kinase gene. “It’s pretty exciting,” said Dr. David Carbone, a lung cancer specialist at Vanderbilt University, one of the sites that tested the drug. Only a small share of lung cancer patients have the gene mutation this drug targets, “but for those people it makes a huge difference,” he said. The FDA expedited approval after Pfizer released data in June showing that 88 percent of patients in one study saw their tumors shrink at least somewhat after one year on Xalkori. In a second study, 54 percent of patients had their tumors shrink by about a third or more and 83 percent had tumors shrink somewhat. Although some may scoff at the fact that it can be used on less than 7% of those diagnosed, you wouldn’t be skeptical if you were in that group. The news is exciting for seniors because these drugs are pill-based and don’t require the use of extended chemotherapy treatments. These are extremely draining and some seniors just can’t tolerate aggressive therapies like this. If an individual is diagnosed with breast cancer, and the individual has an ulcer, can s/he still receive treatments such as chemotherapy, radiation, etc? How does the ulcer affect the treatment options, if at all? What other methods are available? Please be as specific as possible to help me understand.

This is no place to go for a medical consultation. You need to find a source for medical consultation that will give you some confidence in the findings.

An ulcer is no obstacle for other treatments, but the ulcer also needs to be treated.

I have had experience with various medical resources including the Mayo Clinic, Sloan Kettering, University of Chicago, etc.

From personal experience, the resource that provides the most competent cancer diagnosis and treatment is the M.D. Anderson Cancer Center in Houston, Texas.

I would urge you to get a referral and make an appointment as soon as you can.

Today, a family member told me what happened today with her patient. Her patient was a 31 year old, white male who was admitted for stomach pain, contractions and bulging. After conducting some tests, she found out that he had stomach cancer. Later she realized that his cancer had spread to his lungs, and could not be removed through surgery. When the distressed patient asked her what to do next, she assigned him for chemotherapy. However, after a few hours, his vessels clotted and he was sent into emergency care. Then it was discovered that he could now not be treated with chemo. When he asked my family relative what was the next step, she told him clearly that he had about two weeks remaining. From hopeful cure for this young 31 year old male, his situation worsened to this dire extent. He started crying and said that his family had just settled and he had two small kids. He said that his job was barely sufficient to raise his family, and without him, his family would be destroyed. After a few hours, his situation worsened further and after barely making through, his lower body was completely paralyzed. His new life term was shortened to a few hours as his situation worsened exponentially. His family came over, and his young kids had no idea about his condition. His wife was very supportive and both were weeping. I don’t know if the patient is still alive because my relative had to leave as her shift ended; but the condition of this man is not a rarity, for there are 11,000 beds in my relative’s hospital and all of them are filled with cancer patients. What is surprising is the massive number of young people with cancers who had promising lives, cut short by this unexpected, horrible disease. I am very humbled by this.
This man might be diagnosed with cancer, but your death, too, is highly probable at any given moment regardless of your age or past medical history.

I wouldn’t say I believe in God because I’m afraid of death. I find the process of death amazing. This life seems meaningless when practicality hits. The fact that we aren’t immortal. We are powerless. Sometimes, even doctors can’t save someone. We are humans. We are fallible. There are times when we can’t do anything about it. We are so.. limited. Death is inevitable. It IS frightening. How can you deny?

Atheists– don’t take this offensive in any way. I just wanted to know your view of death. I’ve seen atheists on here that are apathetic to this topic. How can you be apathetic? I think, after you’ve seen death from close, there is hardly any way one can be apathetic to this topic. It’s amazing. Death is amazing. Young people die. Healthy people die. The whole process of death is amazing.

It doesn’t. marijuana kills brain cells is based on research done during the second Reefer Madness Movement. A study attempted to show that marijuana smoking damaged brain structures in monkeys. However, the study was poorly performed and it was severely criticized by a medical review board. Studies done after wards failed to show any brain damage, in fact a very recent study on Rhesus monkeys used technology so sensitive that scientists could actually see the effect of learning on brain cells, and it found no damage.

Nothing good comes from marijuana.

No it doesn’t. There has never been one reported death FROM marijuana.

Yes, but only in excessive use. Such as 5 or 6 blunts a day, which is rarely done and prolonged smoking results in minimal lung damage.

I believe the government made it illegal for one reason (in my theory) Money. See, since weed can be naturally grown like any other plant, it would be easy for cancer patients to get their hands on a natural remedy and cure their cancer, but because of this, the income made at local pharmaceutic retail stores would plummet, thus creating a drop in money: something the FDA doesn’t want.

So you see it is not really that bad, so why do people have such a negative perspective on it?

Medical View of Mesothelioma

More than 8 million people have been exposed to asbestos and are at risk of succumbing to mesothelioma. Mesothelioma is caused when the mesothelium cells start to divide rapidly leading to cancer. The only known cause of mesothelioma is exposure to asbestos fibers. The disease has a very long latency period, sometimes even around 60 years. The disease has no known cure and death becomes inevitable.

It is also very hard to correctly diagnose the disease in the early stages, as the symptoms of the disease are similar to other non-malignant diseases like pneumonia. The asbestos fiber, which is light and invisible, is released during mining and manufacture of asbestos. It is inhaled easily through nose and mouth and affects the mesothelium lining of the body part that it comes into contact with. This sets off the mesothelium cells rapid division operation, resulting in tumors. The mesothelium lining can be found in the organs like lungs, abdomen and heart.

Depending on this, mesothelioma can be classified into three categories:§ Pleural mesothelioma, which affects the lungs,§ Peritoneal mesothelioma, which affects the abdomen, and§ Pericardial mesothelioma, which affects the heart.

Due to the long latency period of the disease, people who have worked in asbestos-related factories in their youth may contract the disease in their old age during or after retirement. Many of them may have shifted many jobs by this time and they do not relate their condition to asbestos exposure. Thus, they do not consider mesothelioma as an occupational disease. They face huge sorrow at having to part with their loved ones in such a tragic way.

Life of Jack Daws

Jack Daws was working as a tutor in the rural regions of Pennsylvania. He had a beautiful wife and a handsome son. He had what is called as a perfect family! They were always supportive and co-operative. They had enduring love and constant hope on each other.

Jack Daws loved his job as he loved kids of all ages and all sizes. He was the coach of the local soccer team during weekends. When Jack was around 55 years, he was planning on getting a retirement from the school where he tutored in another year or two.

At this crucial point of time, Jack slowly started realizing something was wrong with his health. He felt a constant pain in his chest and severe shortness of breath. He, his family and his doctor dismissed all these symptoms as a cause of aging, initially. But, slowly, his condition started to worsen. He felt as if a heavy weight has been placed on his chest. Also, he started losing weight rapidly. By now, the doctor also started sensing that something was wrong with Jack. The doctor conducted a whole lot of tests on Jack. One of the tests that were conducted was to study the fluids & cells in his lungs. After this test, the doctor felt that it appeared cancerous. Further tests followed to make the right diagnosis. It was, finally, confirmed. Jack suffered from ‘Pleural Mesothelioma’,I.e. mesothelioma affecting the lungs. Doctors felt that Jack had only another year or two of life on earth. Jack started analyzing how he contracted mesothelioma. He had never worked in any asbestos-related industries. Slowly, it dawned on Jack. Lots of construction and painting work used to be done on in his school.

Due to his long career at the school, he was constantly exposed to asbestos dust, which was used during the painting work. The company, which did the job for the school, did not take any safeguards to protect, anyone. Jack was advised to initiate proceedings against the company. The company did not disclose the dangers of asbestos and behaved in a selfish profit-oriented manner.

When Jack initiated proceeding against the company, it was found that lots of people have already sued the company for the same reason and have won their cases. Thus, the Company did not want to go into lengthy and expensive legal proceedings. They agreed to go in for an out-of-court settlement. After negotiations, the company paid Jack around a million dollars for the health damages he suffered and around two million dollars for his family-dependents. Jack underwent treatment at a cancer center. The cancer was spread all around his body and surgery was ruled out for him. Jack underwent chemotherapy and lived a life of suffering for the next two years. After that he succumbed to the disease. However, his soul will rest in peace as he has provided a safe and comfortable settlement for his loving family members.

I am doing a research which is related to bone remodeling and effect of cancer on the bone.
I want to know what happens in the bone when there is cancer from the scientific / biological point of view.

What does the cancerous cell / tumor do? What changes in the bone?
How the function of Osteoblasts and Osteoclasts change? What changes around the cancerous cell and how?
How the cancer progress? What would be the rate of progression?
Also any information about the properties of the cancerous cell / tumor itself will help (

One of the most common diseases that are making headlines today is cancer. In the last few years the numbers of patients have increased and at the same time cure too has been advanced and assuring. You will notice that presently people are much more educated about this disease, which once was known to be fatal and so tend to discover it at the early stages. There are many types of cancers, which are basically cells that form a tumor by prying the human body to devastate tissues and spread to various parts of the body. One such form of cancer is Mesothelioma which is medically known to be instigated by exposure to asbestos that tends to destroy the membrane which wraps the crucial organs of the human body. This membrane plays a vital part by oozing greasy types of solutions in order to facilitate the smooth movement of the organs in our body.
